Create and Manage Questionnaire Templates

Build multi-section questionnaires with required questions and file-upload responses, reorder sections, and duplicate or archive templates.

Templates > Questionnaires

The Questionnaires tab is where you build structured forms you send to couples to collect details - guest counts, shot lists, dietary requirements, file uploads, anything you can think of. Each template is organised into sections of questions, sent from a booking or fired automatically from a workflow.

Structure

  • A questionnaire template has a name and one or more sections.
  • Each section has a name, an optional description, and one or more questions.
  • Each question has its own text, a mandatory flag, and a response type - either short text or a file upload.

You can drag and drop both sections and questions to reorder them.

Create a questionnaire template

  1. Open Templates and click the Questionnaires tab.

  1. Click Create Questionnaire in the top-right.

Create Questionnaire opens a blank questionnaire builder.
  1. Enter a Template Name.

Give the template a name you'll recognise later.
  1. The form starts with one Untitled Section containing one default question. Click into the section title to rename it, add a description if you want one, then fill in the first question.
  2. Use + Add Question to add more questions to that section. For each question, set its Mandatory flag and its Response Type.

  1. Click Add Section to add another section. You can have as many sections as you need (at least one is required).
  2. Drag a section by its handle to reorder sections; drag a question to reorder it within its section.
  3. Click Save.

Fields

  • Template Name (required) - your label for the questionnaire.
  • Section Name (required) - the title of each section. You need at least one section.
  • Section Description (optional) - a short note shown to couples above that section's questions.
  • Question Text (required) - the question itself.
  • Mandatory (optional, off by default) - when on, couples can't submit the questionnaire until this question is answered.
  • Response Type (required, defaults to Text) - pick Text for a free-text answer, or File Upload to let the couple upload a file as their answer.

Edit a questionnaire template

  1. From the Questionnaires list, click the row's Actions menu and choose View / Edit.

  1. Click Edit at the top.
  2. Add, remove, or rewrite sections and questions. Drag to reorder.
  3. Click Save. Cancel with unsaved changes shows an "Unsaved changes" prompt.

Duplicate a template

Use Duplicate to start a new template from an existing one - handy when two questionnaires are mostly the same.

  1. Open the row's Actions menu and choose Duplicate.

  1. Confirm in the dialog. A copy is created in your library - edit it independently.

Archive (and unarchive)

Archived questionnaires drop out of the picker when sending a questionnaire from a booking. They stay in your library.

  1. Open the Actions menu and choose Archive. The confirmation reads "Archive Questionnaire".

  1. Switch the Status filter to Archived to find them later. Choose Unarchive to bring one back.

Delete a questionnaire template

Permanent. Asks for confirmation.

  1. Open the Actions menu and choose Delete.
  2. Confirm in the dialog ("Are you sure you want to permanently delete this questionnaire? This action cannot be undone.").
